Job summary

Dorothy House is seeking a Registered Nurse to join our Community Team, providing holistic, person-centred care to patients with life-limiting illnesses in community settings. This internal opportunity supports patients and families with assessment, symptom management, planning and multidisciplinary collaboration.

You will work with Community Nurse Specialists and other health professionals, maintain records, contribute to quality improvement, and support education for students and volunteers;

Qualifications

• First Level Registered Nurse with a relevant post-basic qualification.
• Clinical experience at Band 5 level or equivalent.
• Excellent communication and interpersonal skills, particularly with complex care needs.
• Experience working collaboratively within a multidisciplinary team.
• Evidence of continued professional development.
• Full UK driving licence with access to a vehicle.
• Experience managing a caseload independently in a clinical setting.
• Knowledge of palliative and end of life care principles.
• ENB 998 or equivalent teaching experience.
• Community nursing experience or community-based qualification.

Responsibilities

• Undertaking assessments of patients with palliative and end of life care needs in homes, clinics and virtual settings.
• Providing holistic, person-centred care addressing physical, emotional, social and spiritual needs.
• Contributing to complex symptom management and delivery of high-quality palliative care.
• Developing and implementing care plans with patients and families and healthcare professionals.
• Working closely with Community Nurse Specialists, GPs, and the wider team.
• Supporting bereavement assessments and referrals to Dorothy House services.
• Participating in the Dorothy House Advice Line rota after training and competencies.
• Supporting education, mentorship and learning opportunities for students, colleagues and volunteers.
• Maintaining accurate patient records and contributing to audit and quality improvement.
