Job summary

Central London Community Healthcare NHS Trust (CLCH) is seeking a Band 6 Deputy Team Lead for the Dacorum Community Nursing Team, based at The Forum Marlowes, Hemel Hempstead. The role involves co-leading a neighbourhood team, managing a patient caseload, and delivering evidence-based care in patients’ homes and residential settings.

Qualifications

• Registered Nurse with full UK NMC registration.
• Experience in community nursing.
• Ability to lead and supervise a nursing team.
• Valid UK driving licence and access to a car for business use.

Responsibilities

• Set and maintain standards of care; deploy and supervise the nursing team.
• Provide leadership to community nurses across localities.
• Assess, plan, implement and evaluate patient care at home and in care homes.
• Manage a defined caseload and documentation within records.
• Develop and maintain team competencies and supervision.
• Co-lead a neighbourhood team with support from Team Leaders.
• Travel to patient homes as part of community role.
