Job summary

Central London Community Healthcare is seeking a Band 6 Deputy Team Lead for the Dacorum Community Nursing Team. This role involves co-leading a neighbourhood team, coordinating care in patients' homes and residential settings, and working closely with experienced Team Leaders to improve patient outcomes.

You will be responsible for assessing, planning, implementing and evaluating evidence-based care for patients in the community, including travel between homes.

Qualifications

• Registered Nurse with Band 6 level experience.
• Experience leading community nursing teams.
• Strong communication and collaborative skills.
