Job summary

Sirona care & health in Bristol, North Somerset, or South Gloucestershire seeks a Senior Community Nurse to provide patient-centred care in the community and develop leadership capabilities within a supportive team.

You will lead clinical practice, supervise junior staff, and contribute to integrated neighbourhood services delivering holistic assessments and tailored care plans.

Qualifications

• Senior Community Nurse with leadership responsibilities.
• Experience in community nursing and patient-centred care.
• Commitment to developing leadership and management skills.

Responsibilities

• Complete holistic assessments of service users.
• Develop and deliver person centred care plans.
• Line manage junior staff and support their development through supervision, 1:1s and training.
• Undertake training within a robust competency framework to deliver a competent clinical service.
• Communicate with the senior leadership team in the locality to ensure smooth delivery of service.
• Identify risks and act to minimise risk to self, service users and colleagues.
• Coordinate shifts and delegate/allocate care to the team per policy.
• Act as escalation point for staff reporting issues or deteriorations.
• Pursue further training to develop extended skills and enhance patient care.

Our Integrated Neighbourhood Teams are made up of nurses, therapists, paramedics and support workers who work in an integrated way to support service users on their healthcare journey. We would love for you to come along for the ride.

• Complete holistic assessments of service users.
• Develop and deliver person centered care plans.
• Line manage junior staff and contribute to their development through supervision, 1:1s and training.
• Undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and a deliver a competent clinical service.
• Communicate with the senior leadership team in the locality to ensure smooth delivery of service.
• Identify risks and act accordingly to minimise risk to self, service users and colleagues in line with Organisational policy and procedures.
• Coordinate shifts and delegate/allocate care to the team in accordance with policy.
• Act as a point of escalation for staff to report to on discovering any issues or deteriorations.
• Undertake further training as required in order to develop extended skills and enhance patient care.
