Sirona care & health is seeking a Senior Community Nurse to join our South Gloucestershire North locality. You will lead community-based care, complete holistic assessments, and develop person-centered care plans with an emphasis on safe, high-quality delivery.

Responsibilities

• Complete holistic assessments of service users.
• Develop and deliver person-centred care plans.
• Line manage junior staff and contribute to their development through supervision, 1:1s and training.
• Undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and a deliver a competent clinical service.
• Communicate with the senior leadership team in the locality to ensure smooth delivery of service.
• Identify risks and act accordingly to minimise risk to self, service users and colleagues in line with Organisational policy and procedures.
• Coordinate shifts and delegate/allocate care to the team in accordance with policy.
• Act as a point of escalation for staff to report to on discovering any issues or deteriorations.
• Undertake further training as required in order to develop extended skills and enhance patient care.
