Job summary

Sirona care & health is seeking a Senior Community Nurse to lead a multidisciplinary team in the South Gloucestershire North locality. You will conduct holistic assessments, design person-centred plans, and support junior staff through supervision and development opportunities.

The role emphasises safety, clinical judgement, and collaborative working within integrated teams across Bristol, North Somerset or South Gloucestershire, with strong development opportunities in a supportive NHS context.

Qualifications

  • Registered adult nurse with BSc or equivalent.
  • Current NMC registration.
  • Evidence of continuing professional development.
  • Ability to lead a team with senior support.
  • Experience supervising staff and students.
  • Experience in community settings, managing frail complex patients.

Responsibilities

  • Complete holistic assessments of service users.
  • Develop and deliver person centred care plans.
  • Line manage junior staff and provide supervision/training.
  • Undertake training within a robust competency framework.
  • Coordinate shifts and delegate care to the team.
  • Act as escalation point for staff on issues or deteriorations.

Our Integrated Neighbourhood Teams are made up of nurses, therapists, paramedics and support workers who work in an integrated way to support service users on their healthcare journey. Complete holistic assessments of service users.

Develop and deliver person centered care plans.

Line manage junior staff and contribute to their development through supervision, 1:1s and training.

Undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and a deliver a competent clinical service. Communicate with the senior leadership team in the locality to ensure smooth delivery of service.

Identify risks and act accordingly to minimise risk to self, service users and colleagues in line with Organisational policy and procedures.

Coordinate shifts and delegate/allocate care to the team in accordance with policy.

Act as a point of escalation for staff to report to on discovering any issues or deteriorations.

Undertake further training as required in order to develop extended skills and enhance patient care.

  • BSc or equivalent experience in Adult Nursing.
  • Current NMC registration.
  • Evidence of continuing professional development and commitment to lifelong learning, Able to effectively lead a team with senior support when required.
  • Able to make clinically reasoned judgements when working with a variety of conditions.
  • Experience of supervising staff and students of own profession.
  • Effective time management and caseload management, Recent relevant experience within the community setting including management of the frail complex patient.
