Job summary

Sirona care & health CIC is recruiting a Senior Community Nurse (Band 6) on a 12-month fixed-term contract (internal candidates secondment possible). The post is part-time with flexible working and 37.5 hours per week, offering NHS terms and pro rata salary to £48,117.

You'll lead integrated neighbourhood teams, conduct holistic assessments, develop care plans, supervise junior staff and contribute to high-quality, patient-centered community care across Bristol, North Somerset or South

Qualifications

  • Registered nurse with current NMC registration.
  • BSc or equivalent in Adult Nursing (or equivalent).
  • Commitment to continuing professional development.

Responsibilities

  • Perform holistic assessments of service users.
  • Develop and deliver person-centred care plans.
  • Line manage junior staff and support their development through supervision and training.
  • Coordinate shifts and allocate care within policy.
  • Escalate issues to senior leadership as needed.

Our Integrated Neighbourhood Teams are made up of nurses, therapists, paramedics and support workers who work in an integrated way to support service users on their healthcare journey. We would love for you to come along for the ride.

This is a 12 month secondment

Main duties of the job

Complete holistic assessments of service users.

Develop and deliver person centered care plans.

Line manage junior staff and contribute to their development through supervision, 1:1s and training.

Undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and a deliver a competent clinical service.

Communicate with the senior leadership team in the locality to ensure smooth delivery of service.

Identify risks and act accordingly to minimise risk to self, service users and colleagues in line with Organisational policy and procedures.

Coordinate shifts and delegate/allocate care to the team in accordance with policy.

Act as a point of escalation for staff to report to on discovering any issues or deteriorations.

Undertake further training as required in order to develop extended skills and enhance patient care.

Qualifications
  • BSc or equivalent experience in Adult Nursing
  • Current NMC registration
  • Evidence of continuing professional development and commitment to lifelong learning
Skills, knowledge and experience
  • Able to effectively lead a team with senior support when required
  • Able to make clinically reasoned judgements when working with a variety of conditions
  • Experience of supervising staff and students of own profession
  • Effective time management and caseload management
  • Recent relevant experience within the community setting including management of the frail complex patient
  • Experience of supervising staff and students from other professions

You must have appropriate UK professional registration.
