Job summary

The post requires an independent prescribing qualification or progress towards it, HEAB 358 LTCM module, and excellent communication, problem‑solving and decision‑making skills. The role supports expanding proactive frailty care with the neighbourhood team.

Qualifications

• NMC registered nurse with current UK registration.
• Post-registration qualification such as District Nursing or gerontology in long-term conditions.
• Desirable MSc Nurse Practitioner or prescribing qualifications.

Responsibilities

• Take a comprehensive history and perform examinations to inform personalised care plans.
• Integrate care across settings to prevent duplication and delays.
• Develop the therapeutic relationship through active listening and understanding.
• Provide education to empower patients and families in care planning.
• Contribute to policy development and guideline implementation.
• Develop a local area of expertise and promote evidence-based practice.
