Cornwall Partnership NHS Foundation Trust in Cornwall and the Isles of Scilly is seeking a Senior Nurse to work with the Falmouth and Penryn Community Matrons, delivering care at home to people with complex long-term conditions.
The post requires an independent prescribing qualification or progress towards it, HEAB 358 LTCM module, and excellent communication, problem‑solving and decision‑making skills. The role supports expanding proactive frailty care with the neighbourhood team.
We are looking for a Senior Nurse, experienced working with patients in the community with complex long-term conditions.
You will join with our Falmouth and Penryn Community Matrons who, as part of the wider Community Nursing Team, work closely across Adult Community Services, Adult Social Care and Primary Care, using enhanced clinical skills to enable patients to remain independent at home for as long as possible and with the aim of reducing the risk of avoidable hospital admissions.
We are looking for someone who has completed the HEAB 358 Advancing Practice in Long Term Conditions Management module or the equivalent. The post holder should be an independent prescriber or working towards this qualification.
The post holder will need a high level of communication, problem-solving and decision-making skills.
This is an exciting opportunity as you will be expected to work in partnership with others to develop, take forward and evaluate direction, policies and strategies as we seek to expand our proactive frailty approach within the Falmouth and Penryn Neighbourhood Team.
Please note: If you are an internal candidate and wanting to apply for this position as a secondment opportunity, before applying please ensure you have discussed the post with your current line manager and obtained their agreement to support your release for the duration of the secondment.
