Job summary
OneMedical Group in Minehead invites applications for an experienced Community Matron to join our primary care team at Minehead Medical Centre and in patients' homes.
This part-time role (20 hours; pro‑rata for less than 40) focuses on proactive, frailty-focused care, complex long-term conditions, and polypharmacy reviews, with EMIS used for documentation.

Qualifications
- Registered Nurse with valid NMC registration.
- Significant post-registration experience in community or primary care.
- Experience of multi-agency, multidisciplinary working.
- Knowledge of current NHS policy, community nursing models, and primary care priorities.
Responsibilities
- Provide advanced clinical assessment and management of adults with complex, long-term conditions in homes, care homes and community settings.
- Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
- Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations.
