Senior Clinical Assessor/Physiotherapist - SPA - band 7
Full-time - 37.5 hours per week
Bromley Healthcare has a single point of clinical access within its Care Coordination Centre. Core to this operation is a team of Clinical Assessors made up of both nurses and therapists, who will triage all referrals coming into the organisation and then direct to the most appropriate clinician, using the organisations resource as efficiently as possible with the aim of enabling as many people as possible to be cared for within their own homes.
This role will include inreach into the hospital and clinical assessment with patients.
Interviews expected to be held week commencing 9th June 2025
Act as the point of clinical contact for all referrals to Bromley Healthcare. This will include the triage and prioritisation of referrals as well as the provision of specialist advice to both referrers and internal clinicians within Bromley Healthcare (BHC).
Be responsible for managing referrals into the appropriate clinical team,
demonstrating sound clinical decision making and the use of EMIS and the Electronic Patient Record (EPR) systems and processes to provide clear evidence of effective management. Working with the CCC and service leads to effectively co-ordinate staff to provide the most effective patient care.
Manage the safe transfer of care between care providers and makeappropriate referrals to other care providers outside of Bromley Healthcare
Make consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensure clinical reasoning is reflected accurately in case records.
Balance clinical risk and clinical decision making against context, ensuring patients receive safe, effective and timely care.
Whilst utilising your allied health care professional expertise and clinical skills, you will enhance the experience of all appropriate patients under our care to access and use the facilities/services available in the community.
Enable patients to be cared for safely by remotely supporting established care packages at home or in their place of residence.

Enable early supported discharge, prevent inappropriate re-admission to hospital, share nursing knowledge and skills and support the technological advances initiated at Bromley Healthcare.
Support and participate in the development of integrated pathways and care packages which enable patients to be safely cared for outside of the acute hospital setting.
Work closely with multi- disciplinary agencies and Primary Care professionals to provide appropriate and seamless care for patients and their families in the
community.
Provide clinical leadership and clinical management of the Care Co-ordination Centre. Continuously monitor and maintain clinical standards and quality of care.
Monitor effectiveness of BHC protocols and triage requirements and escalate level of care for acutely ill patients where necessary.
To support patient’s access to community health services in the Bromley area (remit may grow to include other areas where BHC holds clinical contracts)
The post holder is responsible to safeguard children, young people and adults at risk.
Key Areas of Work
Maintain a high standard of clinical care for all patients utilising BHC in accordance with organisational guidelines and protocols.
Support care plans for Intermediate care and complex care patients, referring to other services required.
Supports ward staff and community teams via the CCC in the management of sick patients. Providing highly specialist advice on how patients can be best managed in the community.
Acts as the key point of contact with the CFDT to enable the safe and timely discharge of patients from the PRUH.
Develops good system wide working relationships and communicates effectively with clinical colleagues and members of the multi-disciplinary team.
Facilitates the most effective use of the total community resource by assessing capacity and providing effective challenge where required, including utilisation of scheduling tools.
Promotes quality care and non-discriminatory practice as outlined in BHC Policies and Procedures, acknowledge a patient’s personal beliefs and identity.
Clinical Expertise
Demonstrate a level of clinical competence and a high level of specialist knowledge with regard to care delivery, thereby demonstrating clinical credibility and being a credible role model within the team.
Demonstrate professional accountability and adhere to the relevant professional association’s code of conduct.
Take responsibility for the assessment of acutely ill patients who contact the Care Coordination Centre via telephone and video.
Competently assess and triage acutely ill patients intermediate care patients and complex care patients, including the assessment, planning and evaluation of programmes of care.

Salary is determined by NHS experience and current banding/pay point. External applicants will begin at the entry point of the salary scale in line with NHS terms and conditions.
