Job summary

NHS Coventry & Warwickshire Integrated Care Board is seeking experienced registered nurses to join the Children’s Continuing Care service as AACC Senior Nurse Assessors for Children and Transitions. The role focuses on complex assessments, safeguarding, and coordinating care across health, social care and education to ensure the voice of the child and family is central.

Qualifications

• Current NMC registration as a child health nurse or equivalent and a degree in healthcare services.
• Safeguarding Children Level 3 is required.
• Proven CPD and continuing development in child health.

Responsibilities

• Undertake complex, holistic assessments for children and young people.
• Coordinate multi-agency input and Team Around the Child approach.
• Manage defined caseloads and support transitions into adult services.
