We are seeking experienced registered nurses to join our Childrens Continuing Care service as AACC Senior Nurse Assessors for Children and Transitions. This role offers the opportunity to improve outcomes for children and young people with complex health needs, keeping their voice and the views of their families central to decision-making. You will undertake complex assessments and reviews, coordinate multi-agency input, manage a caseload and support young people to move safely and smoothly into adult services. We are looking for skilled, organised and compassionate clinicians with experience working with children and who can work autonomously, chair complex discussions and build strong relationships across agencies.
This role will require weekly office-based working in line with the hybrid working policies set out for each ICB, with office bases available in Coventry, Warwick, Worcester and Hereford for all staff to use. While this role is expected to be place-based, travel will be required across the cluster and to care settings as needed.
NHS Coventry & Warwickshire ICB and NHS Herefordshire & Worcestershire ICB are working together as a cluster with a shared management team. You will undertake complex, holistic assessments and reviews in line with the National Framework for Children and Young Peoples Continuing Care, and prepare and present assessments to the Childrens Continuing Care Panel. You will manage a defined caseload and support the commissioning, monitoring and review of individualised packages of care.
Working with children, young people, families and partners across health, social care, education and the voluntary sector, you will coordinate a strong Team Around the Child approach and ensure care is integrated and outcome-focused. You will contribute to planning timely transitions into adult services through joint assessment and coordinated care. A key part of the role will be contributing to safeguarding, governance, audit, learning and service improvement, as well as providing peer support or deputising for the Clinical Team Manager when required.
