Coventry & Warwickshire Integrated Care Board is seeking a Children's Complex Case Manager to oversee a caseload of paediatric patients with complex healthcare needs, coordinating care across teams and services to deliver high-quality, personalized support.

You will lead care planning, collaborate with families, social services, and education providers, stay updated with best practices, and contribute to continuous quality improvement and effective transitions.

Qualifications

• Undertake multi-disciplinary assessments and comprehensive reviews of children with complex healthcare needs.
• Lead and coordinate development of individual care plans tailored to each child.
