Job summary

Central London Community Healthcare NHS Trust is seeking a Registered Nurse to join our Continuing Healthcare and Complex Care team as a Senior Clinical Assessor. You will undertake comprehensive assessments, coordinate care packages, and support people and families through the CHC process with a focus on safe, personalised outcomes.

Qualifications

• Registered nurse on the NMC register with current UK registration.
• Educated to degree level in a relevant field.
• Evidence of continuous professional development.
• Knowledge of the National Framework for NHS Continuing Healthcare and related guidance.

Responsibilities

• Undertake comprehensive clinical assessments to support CHC eligibility decisions.
• Develop and review personalised care packages based on assessed needs.
• Collaborate with individuals, families, multidisciplinary teams, local authorities and care providers.
• Support commissioning and brokerage of appropriate care and monitor its quality and effectiveness.
• Identify and manage clinical risk and respond to safeguarding concerns.
• Explain CHC decisions and appeal routes to individuals and families.
• Monitor care providers and contribute to quality assurance.
• Promote safe, effective, person‑centred care for complex health and social needs.
• Contribute to service improvement and CHC framework applications.
