Job summary

Anglia Case Management (ACM) is seeking an experienced Case Manager to join the Cambridge area, covering Cambridge and surrounding regions. You will manage a caseload of clients with complex needs, coordinating multidisciplinary teams and assessing rehabilitation requirements to develop tailored plans.

The role offers flexible remote working with a preference for full-time but a minimum of three days per week is considered, with regular travel and occasional office attendance for meetings.

Qualifications

• Proven ability to conduct detailed assessments and identify rehabilitation needs.
• Experience developing and monitoring individual case management plans.
• Experience coordinating multidisciplinary teams and commissioning services.
• Strong report writing and documentation skills.

Responsibilities

• Undertake detailed assessments and identify clients’ rehabilitation and support needs.
• Develop and regularly review individualised case management plans.
• Commission and coordinate appropriate rehabilitation services across sectors.
• Coordinate multidisciplinary teams and monitor interventions.
• Produce assessment and review reports and maintain case records.
• Support clients with housing, education, leisure and community needs.
