Active Care Group in the United Kingdom is seeking a Senior Case Manager for the Wigan area. This role combines heavy caseload management with leadership, mentoring, and business development within the ACG Case Management Group.

You will mentor Case Managers, ensure evidence-based, person-centred care, and collaborate with clinicians and referrers to drive clinical excellence while benefiting from extensive professional development opportunities.

Qualifications

• Registered healthcare professional with current registration.
• Minimum 2 years' Case Management experience.
• Experience supervising, mentoring or supporting other professionals.
• Strong understanding of rehabilitation and complex case management.
• Excellent written and verbal communication skills.
• Full UK driving licence and willingness to travel within the region.

Responsabilités

• Manage a caseload of clients with complex injuries and disabilities.
• Provide mentoring, guidance and supervision to Case Managers across the business.
• Develop relationships with referrers, solicitors, insurers and other stakeholders.
• Contribute to clinical governance, audits and quality assurance.
• Support recruitment and workforce planning with the Clinical Lead.
