Job summary

Mereside Medical Practices (Staploe Medical Centre, Cathedral Medical Centre, Haddenham Surgery) is hiring for a permanent, flexible role in primary care. The post involves triage, urgent and long-term care management with NICE-guided practice, and close collaboration across Mereside sites to deliver high-quality patient care.

You should be a qualified Paramedic or Nurse with registration, ideally with 2+ years post-registration experience and related minor illness qualifications.

Qualifications

• Qualified Paramedic or Nurse with current registration.
• Ideally 2 years post-registration experience.
• Advantage: Minor Illness assessment and management qualification.
• Experience delivering holistic care in the community.

Responsibilities

• Triage urgent care requests and manage patient flow.
• Provide holistic care, assessment, diagnosis, and treatment per NICE guidelines.
• Collaborate with care homes and families, and coordinate with multi-disciplinary teams.
• Lead in quality governance, self and peer review and continuous improvement.
• Support lead GPs with routine reviews and chronic condition management.
