Job summary

Havering Crest PCN is expanding its team and seeking an Advanced Nurse Practitioner to work with GP practices and the broader PCN staff. The post involves clinical leadership within a multidisciplinary team, managing undifferentiated and complex patient conditions, and contributing to personalised care and vaccination programmes.

Qualifications

• 5 years post registration experience including 6 months experience in GP practice/ Walk in Centre/ Urgent Care Centre
• Nurse prescribing experience
• Experience managing chronic disease
• Clinical leadership experience
• Audit, research and service development experience
• Proficiency with EMIS/SystmOne or similar patient record systems
• Non-Medical Prescriber status
• Level 3 Extended brief interventions (NICE guidelines)

Responsibilities

• Assess, diagnose, plan, implement and evaluate treatment/interventions for patients with undifferentiated diagnoses
• Clinically examine and assess patient needs and plan
• Diagnose and manage acute and chronic conditions, integrating drug and non-drug therapies
• Prescribe and review medications within scope of practice and guidelines
• Lead and support the nursing team, promoting best practice and patient safety
• Develop personalised care plans and coordinate with MDT members
• Manage vaccination programmes and immunisations as appropriate
• Support and lead on risk management, governance and audit processes
