Job summary

The Woodsley & Holt Park PCN is seeking a Lead Nurse to work with the PCN leadership team to support the development and delivery of services across practices. You will empower General Practice staff, mentor Phlebotomists and a Nursing Associate, and collaborate with GP practices and the PCN Manager to deliver PCN objectives at scale.

Applicants must be an NMC Registered Nurse with a strong background in general practice, and an interest in leadership, population health and new primary care

Qualifications

• A good knowledge of, and experience in, general practice.
• An interest in leadership within primary care.
• A good working knowledge and interest in new models to strengthen and improve primary care and ideas on how to implement these.
• An understanding of NHS Long Term Plan and population health management.

Responsibilities

• Demonstrate understanding of, and contribute to, the vision of the PCN Leadership Team.
• Provide leadership and mentoring to the Phlebotomists and a Nursing Associate across the PCN.
• Demonstrate ability to improve quality within limitations of service.
• Review progress and develop plans to achieve results within priorities.
• Follow professional and organisational policies relating to performance management.
• Lead and mentor a team of differing abilities.
