Mersey and West Lancashire Teaching Hospitals NHS Trust is seeking an experienced Advanced Clinical Practitioner to join our General Practice team. The role emphasizes autonomous clinical practice, collaboration across primary and secondary care, and leadership in education, supervision, and audit.

You will contribute to service development and patient-focused care, delivering expert decision making while supporting staff and improving outcomes across the Trust's network.

Formación

• Registered health professional with non-medical prescribing responsibilities.
• First level registered nurse with UK registration.
• First Degree (or equivalent experience).
• Post registration course in specialty.
• Immediate Life Support.

Responsabilidades

• To be professionally and legally responsible and accountable for all aspects of own work, ensuring a high standard of clinical care.
• To act as the patients advocate at all times.
• To ensure appropriate supervision and clinical support is sought when required and always work within own scope of professional practice.
• To have the skills to manage a diverse and often complex caseload efficiently with clinical priorities.
• To prescribe medications under supervision from an agreed formulary.
• To receive referrals from and make referrals to consultants, GPs and other healthcare professionals.
• To promote a clinical environment conducive to high quality patient care and service delivery.
• To assess, plan and implement care interventions for patients requiring complex care and monitoring within scope of practice.
• To modify treatment plans in consultation with patients and multidisciplinary team.
• To develop learning goals with medical and academic supervisors and pursue advanced practice skills.
• To maintain accurate patient notes and discharge planning as required.
• To contribute to service development and protocol/guideline creation and evaluation.
