Job summary

Dovetail and Slate is seeking a Transition Practice Lead in Surrey to oversee and develop transition processes for learners with SEND, EHCPs, and other barriers to learning. You will lead transition, assessment and admissions to ensure learners receive the right support from day one.

Role includes coordinating EHCP consultations, developing support plans, and leading a team of Transition Officers while collaborating with Local Authorities, schools and families.

Qualifications

• Experience working with SEND learners in education or relevant support setting.
• Knowledge of EHCPs, SEND transition and the SEND Code of Practice.
• Experience coordinating EHCP consultations or statutory processes.
• Previous line management or team leadership experience.
• Experience working with Local Authorities, schools, SENCOs and families.

Responsibilities

• Lead transition, assessment and admissions processes to ensure learners receive the right support from day one.
• Coordinate and quality assure EHCP consultations and statutory processes.
• Develop effective transition and learner support plans.
• Lead and support a team of Transition Officers.
• Work with Local Authorities, schools, SENCOs and families to ensure smooth transitions.
• Support EHCP annual reviews and compliance with the SEND Code of Practice.
• Monitor learner needs, provision and High Needs funding.
• Drive high-quality, inclusive practice across the organisation.
