Job summary

WeDoData is seeking a SEND Transition Worker for a further education client in West London. You will work with young people from Year 9 onwards to prepare them for college, arrange visits and tasters, and liaise with schools and Local Authorities to support outcomes.

There is an administrative element, including EHCP information, learner records, deadlines, and review processes. You must have SEND experience in education and strong IT/admin skills.

Qualifications

• Experience of working with children or young people with SEND in education.
• Organised, approachable and confident communicating with learners, families, schools and other agencies.
• Good IT and administrative skills.

Responsibilities

• Help young people from Year 9 onwards prepare for college and understand what to expect.
• Arrange visits and tasters, attend EHCP reviews, liaise with schools and Local Authorities.
• Help learners think about their options after college and manage EHCP information.
• Maintain learner records, monitor deadlines and support the review process.
