Job summary

Education Partnership North East (EPNE) is recruiting for a role focused on early, collaborative transition planning for students with SEN. You will work with schools to assess needs, design support, and prepare learners for college life, including EHCP processes and multi-agency collaboration.

Join a dedicated team driving better outcomes for young people, with a strong emphasis on achieving smooth transitions and improved progression pathways through planned support.

Qualifications

  • Level 2/3 qualification in learning support or equivalent.
  • Level 2 literacy and numeracy.
  • Strong knowledge of SEN and EHCP processes.
  • Experience working with schools and external agencies.
  • Ability to assess needs and plan effective support.
  • Clear communication and relationship building skills.
  • Strong organisational and record keeping skills.
  • Ability to work independently and as part of a team.
  • Experience supporting transition into further education.
  • Familiarity with college systems for student tracking.
  • Experience contributing to EHCP reviews and planning.

Responsibilities

  • Work in partnership with schools to plan and deliver early transition support
  • Assess student needs and develop tailored transition plans
  • Manage a caseload of SEN students and track progress
  • Contribute to EHCP processes, meetings, and reviews
  • Liaise with families, staff, and external agencies
  • Maintain accurate records and update relevant databases
