South Tyneside HealthCollaboration manages two general practices, one in the beautiful coastalvillage of Whitburn and another, a developing training practice in centralJarrow, with a combined list size circa 8,500.
We host 3 PCNs within SouthTyneside as well as deliver several primary care services such as mentalhealth, spirometry, Covid Vaccinations, Admiral Nursing service and Health and Wellbeingoutreach.
Due to the expansion of patientservices, we are looking for a salaried GP to work 4-8 sessions flexibly acrossthe organisation. The successful candidate will be predominantly based withinour practices, with the potential to develop their career into other areaswithin STHC. We can be flexible with number of sessions and working days forthe right candidate.

Duties and Responsibilities
As post holder you will beexpected to support STHC in providing all services outlined in the GMScontract, including work outside of core hours (Enhanced Access).
You will be expected to supportand input into areas of clinical governance, good practice, quality outcomesframework, enhanced services, practice audit, referral review and any otherimprovement plans as determined by the STHC Clinical director.
You are encouraged to utiliseand develop skills for the improvement of services to patients, such as childhealth surveillance, family planning training or any other skills that would berequired for services STHC wishes to sign up to. This will of course be subjectto presentation of the appropriate qualifications.
You will be expected to maintainand keep good standards of computerised clinical records in line with GMCguidance. Full involvement in STHC training and ongoing maintenance of ourstatus as a paperless practice is expected.
You should be competent to workas duty doctor as well as dealing with other daily enquiries coming your way.It is also expected that you may be involved in the monthly care home andpractice MDT meetings. It is expected to be able to complete all work by theend of the working day, for example prescriptions, tasks, lab links andreferrals.
You will be expected to maintainand keep good standards of documentation using EMIS.

The duties of the post are toprovide a full range of general medical services as contained within theNational Health Services (Primary Care) Act 1997 and which will includedomiciliary visits as deemed appropriate. These visits must be triaged and onlyoffered to genuinely housebound patients.

You will be expected to lead inan area(s) of clinical performance and/or service delivery.
1. Review of Job Plan
Your role will be reviewed after3, 6 and 9 months of employment, and will be developed collaboratively betweenthe STHC and yourself. After this time it will be subject to annual review, orwhen there are any significant changes to the work pattern suggested by eitherparty.
2. Scheduling in the Job Plan:
Clinical duties:Youare contracted to provide an agreed number of sessions per week, as per yourcontract, but you will be expected to be flexible, in particular during holidayperiods. You have agreed to deliver an agreed number of sessions per week forEnhanced Hours working.
Clinical duties will includeseeing people with both acute and long term conditions, which will also includereviews based on the QOF
You will be expected to becompetent and confident to deliver face to face, telephone, video and email consultation as the practice provides allof these.
Appointments are scheduled at 10minute intervals (which can be changed to longer if appropriate) and you willprovide face to face consultations in the morning and in the afternoon, wherethis is clinically appropriate, and additional extra consultations where demandis excessive.
You will be required to makehome visits within the session time, however the actual number of house callsis very low and we have specialist care home practitioners that carry out manyvisits that would otherwise have been undertaken by a GP.
You will be expected to deal ona daily basis with telephone queries from patients or other health careprofessionals in your administrative time.
You are expected to follow upresults, x-rays, correspondence related to your own patients and also thosewhich have not been generated by yourself and if necessary, the investigationsand queries related to other clinicians within the practice, however we do havea robust process for the pre-screening and resultant action of allresults/incoming documents so the GP gets very few to act upon.
Referrals should be completed ona daily basis in line with practice policy.
Investigations and referralsshould be completed and associated documentation dealt with within appropriatetimescales.
Reports: You will be expected tocomplete examinations and reports for the benefits agency and those expectedunder the GMS contract on patients known to you.
Additionally you may be expectedto complete reports requested by other parties such as insurance companies,OFSTED and employers.
3. Computerised records
You will be expected to use EMISbespoke software and follow appropriate processes for computerised recordkeeping, including computerised clinical templates and protocols as well asensuring that significant information as determined by the practice is recordedaccurately e.g. all QOF domains. You will take part in reconciling informationinput periodically.
4. Primary care team meetings:formal or informal, essential to the delivery of team based care are held fromtime to time at the surgery and you will have a close working relationship withthe primary care team on a daily basis. You will also be expected to attend anynecessary PCN meetings (PCN West) and attendance at the monthly PracticeEducational Events run by ICB are encouraged. There are quarterly staff meetingat Albert Road surgery and it is expected to attend.
5. Personal Development - STHCis committed to the highest standards of evidence based medicine and supportingcolleagues in achieving this. You will be expected to participate using yourPersonal Development Time (dependant on sessions per week) in discussions onclinical practice standards, developing practice protocols, mutual professionalsupport for the individual practitioners, audit, significant event analysis,meetings with STHC clinical lead, colleagues in the locality, ICB etc. Wherethese occur on an ad hoc basis, adjustments to clinical workload may berequired.
You may be encouraged to becomeresponsible for particular areas of practice development.
You may be encouraged to beresponsible for an area of the Quality and Outcomes Framework (QOF), BOS work,PCN work, IIF and other local initiatives.
6. Additional sessions: STHC mayagree with a practitioner to undertake work which is not specified in his orher Job Plan by way of additional nominal sessions or fractions thereof. Theextra session(s) shall be remunerated on a pro rata basis to a full-timepractitioners salary. Any such agreement shall be reviewed when required but atleast annually and will be terminable at three months notice on either side.
