Job summary

The Bridge Project Bradford is recruiting a full-time Rough Sleeper Recovery Coordinator to join the New Vision Bradford partnership and support people with alcohol and drug issues across the district. The role involves outreach, care coordination, assessments, and developing tailored recovery plans.

You will work within a multi-disciplinary team, travel across the district, and require a valid driving licence with work vehicle access.

Qualifications

• 2 years’ full-time experience working with people with alcohol and/or drug problems and/or people with complex and multiple needs within a 4-year period.
• Experience of case management and delivering care coordination in drug and/or alcohol or related fields.

Responsibilities

• Act as the main care coordinator for an assigned group of service users, coordinating tailor-made packages of care.
• Provide comprehensive assessments and recovery planning.
• Deliver one-to-one key work, psychosocial interventions and harm minimisation.
• Lead outreach to connect with rough sleepers or those at risk and coordinate services.
• Support service users through entry into the service and throughout treatment and recovery.
