Job summary

Barts Health NHS Trust in London is seeking motivated pharmacists to join the rotational Band 7 pharmacist programme across four sites: Royal London, St Bartholomew's, Whipps Cross and Newham hospitals. You will work within the Pharmacy Department delivering medicines optimisation and patient care.

The programme provides ward-based and dispensary responsibilities, with emphasis on medication reconciliation, patient counseling, discharge planning and collaboration with the multidisciplinary team

Responsibilities

• Medicines optimisation across wards and with multidisciplinary teams
• Medication reconciliation and original pack dispensing at ward level
• Counselling patients about their medicines and coordinating discharge medications
• Coordinate supply of discharge medication and maintain high standards of care on the wards
• Work with ward-based teams to continually develop the service and meet needs of patients and Trust
