Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust in Alton is seeking an enthusiastic Band 6 Occupational Therapist to join a rotational Community Rehabilitation Hospital, Hospital at Home and Community Therapy Team service. You will assess, treat and rehabilitate patients following illness or deconditioning, enabling safe discharge and independence at home.

Working within a multidisciplinary team, you will support admission avoidance, provide supervision, and contribute to service

Qualifications

• Appropriate professional qualification (Occupational Therapist) with full HCPC registration.
• Degree level award (level 6) plus post graduate experience.
• Able to demonstrate a good command of the English Language both written and verbal.
• Computer literate in data entry and report writing.

Responsibilities

• Manage complex caseloads across community and rehab settings.
• Undertake holistic assessments and treatment planning.
• Work with multidisciplinary teams to achieve patient-centred outcomes.
• Support hospital avoidance and timely discharge planning.
• Provide supervision for junior staff, assistants and students.
• Participate in service development, quality improvement and clinical governance.
• Contribute to integrated health and social care pathways.
