Job summary

Responsibilities

• Establishes and maintains effective communication with patients, carers and staff.
• Uses OT theory and models to address patient needs and inform decision making.
• Carries out assessments and treatments involving moderate physical effort.
• Plans and implements treatment plans and selects appropriate assessments.
• Ensures timely discharge planning and liaison with other agencies across the patient care pathway.
