Making Space (Warrington) is seeking a Community Navigator for the Cambridgeshire Navigators Service. The role focuses on linking adults with services, offering information, guidance and signposting to help explore options and reduce isolation.

Based predominantly at home with some local travel, the post is part-time with a hybrid pattern. Priorities include supporting independent living and providing empathetic, person-centred guidance.

Qualificações

• Provide one-to-one guidance and information for individuals' needs.
• Assess needs and produce plans to meet outcomes.
• Willingness to work towards NVQ Level 2 in Health and Social Care.
• Align with Making Space values.

Responsabilidades

• Address rising needs of adults in Cambridgeshire by connecting them with local services.
• Offer information and guidance to help clients explore their options.
• Provide support via face-to-face, telephone, virtual appointments and email/text.
• Promote independence and reduce isolation.
