Job summary

Center for Life Management (CLM) offers comprehensive mental health services and a Collaborative Care Community Support Counselor role. This full-time position provides outpatient, community-based support to improve symptoms and daily functioning.

The role involves working with a multidisciplinary team and requires documenting services weekly. CLM supports a Master’s degree path with tuition coverage and a strong benefits package, including health insurance, retirement, and training

Qualifications

• Bachelor’s degree preferred; direct service experience valued.
• Experience with individuals with intellectual disabilities and/or traumatic brain injury preferred.
• Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.

Responsibilities

• Provide psychoeducation using IMR to support goal and coping skills.
• Provide counseling and Functional Support Services in home and community settings.
• Provide Individual Therapeutic Behavioral Services to develop coping strategies.
• Provide Family Training and Counseling to educate natural supports and promote resiliency.
• Provide Medication Training and Support to educate and plan medication use.
• Provide Crisis Intervention when needed to ensure safety.
