Job summary

Center for Life Management seeks a Community Engagement and Outreach Case Manager to perform assessments, develop treatment plans, and provide home and community-based counseling to adults with complex mental health needs. The role emphasizes engagement, crisis management, and coordination with a multidisciplinary team.

Applicants should hold a bachelor’s degree or higher, have experience with mental illness and/or co-occurring disorders, and be proficient with Microsoft Office and EHRs.

Qualifications

• Bachelor’s degree or higher required.
• Experience with mental illness and/or co-occurring disorders preferred.
• ANSA and Eligibility for State Funded Services knowledge preferred.
• Strong clinical judgment in routine, urgent and emergent situations.
• Proficient with Microsoft Office; experience with electronic health records.

Responsibilities

• Provide assessment of client strengths, needs, resources and objectives.
• Formulate treatment plans with problems, objectives and interventions.
• Complete eligibility assessments for State funded services.
• Develop care plans, crisis management, and referrals.
• Engage in outreach, advocacy and coordination of services.
• Use motivational interviewing to support wellness goals.
• Provide IMR-based psychoeducation and supportive counseling.
• Deliver in-home or community-based services as planned.
• Provide Crisis Intervention for acute symptoms when needed.
• Maintain client confidentiality and documentation per agency policies.
