Job summary

Placements24 is seeking a highly organized Remote Case Manager to join our client’s team. You will assess needs, develop care plans, and connect clients with resources from a remote location, ensuring coordinated support for individuals and families in Randburg and beyond.

The role requires excellent communication, problem-solving, and strong administrative skills, with a track record of managing caseloads remotely and using case management software.

Qualifications

• Bachelor's degree in Social Work, Psychology, or a related human services field.
• Minimum of 2 years of experience in case management or a similar role.
• Proficiency in using case management software and virtual communication tools.
• Strong understanding of community resources and social service delivery systems.
• Excellent organizational, time management, and problem-solving abilities.
• Demonstrated ability to work autonomously and manage a caseload effectively from a remote setting.

Responsibilities

• Conduct thorough needs assessments for clients via virtual meetings and phone calls.
• Develop, implement, and monitor individualized case management plans.
• Coordinate and arrange services with various internal and external providers, such as healthcare, housing, and employment agencies.
• Provide advocacy and support to clients navigating complex systems and accessing resources.
• Maintain accurate, up-to-date client records and progress notes in the designated digital system.
• Regularly review case plans and client progress, making adjustments as needed to ensure optimal outcomes.
