Job summary

Codi Group is seeking a Complex Care Project Worker (Relief) to empower residents at Ty Anwen, Bedwas, Caerphilly. You will help develop and review person-centred care plans, support daily living, and promote independence in a 24/7 residential setting.

You’ll join a dedicated team, undertake role-specific training, and liaise with Social Services and other agencies. The role is flexible and requires commitment to ongoing training.

Qualifications

• IT literacy demonstrated.
• GCSE Maths & English at Grade C or above (or equivalent).
• Experience working as part of a team.
• Minimum of 2 years’ experience with mental health or complex care needs.

Responsibilities

• Support individuals to live independently within a care setting.
• Support, prompting and administering medication.
• Engage individuals in social activities.
• Complete and update support plans and risk assessments.
• Undertake relevant training for the role.
• Liaise with Social Services, Benefits Agencies, Housing and other relevant agencies.
• Organise group activities at Ty Anwen or in the community.
• Provide emotional support to individuals.
