Job summary

Codi Group in Bedwas, Caerphilly region is seeking a Complex Care Project Worker to empower individuals on their recovery journey and support high-quality, person-centred care plans. You will work with a dedicated team to reflect each person's needs, preferences and aspirations.

The role involves delivering care across a 24/7 service with day, evening, weekend, and night shifts, and you will contribute to a recovery-focused model of care with strong teamwork and training opportunities.

Qualifications

IT literacy
GCSE Grade C or above in Maths & English (or equivalent)
Experience working as part of a team
Minimum of 2 years' experience working with individuals with mental health or complex care needs

Responsibilities

Supporting individuals to live independently within a care setting.
Support, prompting and administering medication.
Engagement in social activities
Completing and updating support plans, risk assessments and risk management plans.
Undertake relevant training for the role.
Liaise with Social Services, Benefits Agencies, Housing department and other relevant agencies
Organising group activities with the individuals at Ty Anwen or in the community
Providing emotional support to the individuals
