Job summary

Powys Teaching Health Board is seeking a motivated Rehabilitation Therapy Assistant to join the Community Therapy Team in the Therapy Led Reablement service. You will help deliver patient-centred rehabilitation in home settings, working with occupational therapists, physiotherapists and other professionals.

You will contribute to rehabilitation, support daily living activities, and follow therapy plans while maintaining clear records.

Qualifications

• Excellent communication and teamwork abilities are essential.
• Experience in healthcare or therapy settings is desirable but not required.
• Able to work flexibly across shifts and weekends as part of a 7-day rota.

Responsibilities

• Deliver rehabilitation through therapy interventions in patients' homes.
• Support personal care, cognition, mobility, and daily living activities.
• Implement rehabilitation plans delegated by therapists.
• Maintain accurate documentation and communicate with patients and families.

An exciting opportunity has arisen for an enthusiastic and motivated individual to join our Community Therapy Team as a community basedRehabilitation Therapy Assistant within our Therapy Led Reablement service.

As a vital part of our multi-disciplinary team, you will contribute to delivering a patient-centred rehabilitation service, working closely with occupational therapists, physiotherapists, and other healthcare professionals.

• Deliver rehabilitation through therapy interventions, which may include support with personal care tasks, cognition, mobility and activities of daily living.
• Carry out rehabilitation plans as delegated by occupational therapists and physiotherapists.
• Maintain accurate documentation and communicate effectively with service users, families, and the therapy team.
• Be flexible to support the team during peak service times.
• Participate in the 7-day service rota, including weekends, delivering rehabilitation between 8am and 8pm. Mix of 8am-2pm, 2pm-8pm and 8am-8pm shifts.
• You will be required to work 1 in 3 weekends.

• Organised with good time management skills.
• Experience in a healthcare or therapy setting is desirable but not essential.
