Job summary

Active Care Group is recruiting a Rehabilitation Assistant for the Chislehurst Neurological Rehabilitation Centre in Greater London. You will support HCPC-registered therapists with individual and group rehabilitation sessions, and help patients with mobility, personal care and daily activities.

The role follows a 7-day rota with weekend shifts. The organization offers extensive training, development opportunities and a supportive, multidisciplinary team in a leading UK complex care provider.

Qualifications

• Experience in healthcare, rehabilitation or therapy environment.
• Understanding of disability, neurological conditions and rehabilitation principles.
• Ability to work flexibly across a 7-day rota and start shifts on time.
• Excellent communication, interpersonal and team-working skills.
• Willingness to undertake specialist training as part of induction.

Responsibilities

• Deliver individual and group rehabilitation programmes as prescribed by HCPC-registered therapists.
• Undertake treatment sessions or components of treatment under guidance following care plans and risk assessments.
• Provide feedback to the therapeutic team on adjustments to treatment programmes and goal attainment factors.
• Support patients with mobility, positioning, transfers, self-care and functional activities.
• Assist with personal care and rehabilitation activities during the first part of the shift with nursing/HCAs.

We'd like you to join us as a Rehabilitation assistant at our Chislehurst Neurological Rehabilitation Centre. At our Chislehurst Neurological Rehabilitation Centre, we provide specialist post-acute rehabilitation, as well as management of long-term neurological conditions to adults over the age of 18. We aim to develop person-centred, evidence-based rehabilitation care plans to maximise independence and recovery. We offer slow stream rehabilitation providing the opportunity for residents to continue their rehabilitation through an adjusted intensity programme tailored to their specific needs. This flexibility accommodates resident’s current needs and strives towards achieving their rehab goals.

What you’ll be working:

Your hours are based on a rota working every other weekend. Full time- 08:00am to 20:15pm 42 hours per week £31,207.59 (FTE) per annum pro rata.

What you’ll be doing:

• Deliver individual and group rehabilitation programmes as prescribed and delegated by HCPC-registered therapists.
• Undertake treatment sessions or components of treatment under guidance, following therapy care plans and risk assessments.
• Feed back to the therapeutic team regarding adjustments to treatment programmes and factors impacting goal attainment
• Support patients with mobility, positioning, transfers, self-care and functional rehabilitation activities as directed.
• Assist with personal care and rehabilitative activities during the first part of the shift in collaboration with Healthcare Assistants and nursing staff.
• Observe, monitor and accurately report patient progress, tolerance and response to treatment to the supervising therapist.
• Maintain timely, accurate and confidential clinical documentation.
• Recognise own limitations, maintain professional boundaries and seek advice or escalation when required.

What you’ll have:

Essential

• Experience working in a healthcare, rehabilitation or therapy environment.
• Understanding of disability, neurological conditions and rehabilitation principles.
• Ability to work flexibly across a 7-day rota and commence shifts at ward start times.
• Excellent communication, interpersonal and team-working skills.
• Willingness to undertake specialist and advanced training as part of induction.

Desirable

• Experience working with patients with neurological conditions or brain injury.
• Experience delivering delegated therapy programmes.
• Competency in assisting with mobility, transfers and rehabilitation exercises.
