Job summary

Phyllis Tuckwell Hospice in Farnham and Guildford is seeking a full-time Rehabilitation Assistant to support high quality rehabilitative palliative care in patients' homes and at our sites.

You will work alongside an Occupational Therapist and other Rehabilitation Assistants to help those living with advanced disease engage in meaningful activities, with opportunities for training and career development.

Responsibilities

  • Support high quality rehabilitative palliative care for patients, their relatives and carers.
  • Work primarily in the community, in patients' own homes, plus occasionally at Phyllis Tuckwell sites in outpatient and group settings.
  • Collaborate with an Occupational Therapist and other Rehabilitation Assistants to enable those living with advanced disease or terminal illness to develop skills and confidence.
  • Empower patients to maximise their ability to engage in meaningful activities, promoting quality of life.

Rehabilitation Assistant

Phyllis Tuckwell is looking to appoint a full time Rehabilitation Assistant. The role supports high quality rehabilitative palliative care for patients of Phyllis Tuckwell Hospice.

The successful applicant will be an integral member of our community team and wider multi-disciplinary team. They will work primarily in the community, in patients’ own homes, plus occasionally at our Phyllis Tuckwell sites in outpatient and group settings. The successful applicant will work alongside an Occupational Therapist and other Rehabilitation Assistants to enable those living with advanced disease or terminal illness to develop the skills, knowledge and confidence to actively manage their condition. The role will empower patients to maximise their ability to engage in activities that are meaningful to them, promoting quality of life according to their preferences, needs and choices.

About You:

A successful Rehabilitation Assistant will have:

  • A level 3 qualification in Health or Social Care of equivalent.
  • An understanding of palliative care.
  • Be committed to person centred, high-quality, rehabilitative palliative care for patients, their relatives, and carers.
  • Experience in and be committed to working as part of an effective multi-disciplinary team.
  • Well developed communication skills.
  • Good information technology skills across a range of platforms.
  • Be self-aware and resilient.
  • Have the skills required to work with groups of patients and carers.
