Job overview

This is an exciting opportunity to join the Community Physical Health Team as we move into a new model working closely with our Primary Care colleagues to deliver health assessments for people with severe mental illness. The post is open to qualified Nurse Associates covering Sefton, Liverpool, and Mid Mersey areas.

The post holder will demonstrate excellent clinical and communication skills and be a team player working closely with the Community Physical Health team and Primary Care. You will have a clear commitment to person-centred care and will be able to demonstrate this in your work with individuals, families, and teams. You will be expected to work under the supervision of the band 7.

The post holder will provide physical health assessments, health promotion advice in line with national guidance, and signpost/refer individuals to appropriate services/organisations to meet the needs identified through their health assessment. There will also be a requirement for the successful candidate to work with the team in developing and delivering enhanced support to the identified patient group and monitoring the impact of their interventions on patient outcomes.

Main duties of the job

1. Provide clinical assessment, screening, and intervention for the physical health needs of our client group.
2. Work with qualified nurses in meeting the health outcomes of our clients and provide person-centred intervention and holistic care.
3. Liaise with identified GP practices ensuring the patients' needs are appropriately identified and met.

Detailed job description and main responsibilities

1. Develop understanding of all elements of the nursing process and assist the registered nurse in the ongoing assessment, planning, management, and evaluation of care.
2. Support individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence.
3. Demonstrate the principles of safe administration of medicines in line with trust policies.
4. Perform and record clinical observations including pressure, temperature, respirations, and pulse.
5. Undertake clinical tasks including venepuncture, ECGs, and cannulation (following appropriate training).
6. Accurately record nutritional and fluid intake and output as clinically indicated.
7. Ensure the privacy, dignity, and safety of individuals is maintained at all times.
8. Recognise changing priorities and seek advice from the Registered Nurse as appropriate.
9. Report back and share information with registered nurses on the condition and responses of individuals.
10. Recognise issues relating to safeguarding vulnerable children and adults and report any concerns.
11. Assist in the assessment and management of risk across care environments.
12. Assist in delivering complex care as prescribed by the registered nurse.
13. Maintain an understanding of caring for individuals with particular conditions such as dementia and mental illness.
14. Engage in reflective practice.
15. Demonstrate good understanding of principles of consent and ensure valid consent is obtained.

Person specification

Qualifications

• Nursing Associate Foundation Degree from an approved programme.
• Registered Nurse Associate on the NMC register.
• Additional post-registration qualification relevant to field of practice.
