Job Summary

Registered Nurse – Home Nursing Service – Annualised hours (average 17 hours per week) – Salary £15,972 per annum (FTE £35,233) plus uplift for unsociable hours. Enrol in a passionate team delivering high quality palliative and end‑of‑life care. Excellent communication skills and commitment to team collaboration with specialist palliative care services.

  • Contract: Permanent
  • Hours: Annualised 887 hours per year (average 17 hours per week)
  • Salary: £16,291 per annum (FTE £35,937) plus hourly uplift: £2.59 for 10pm‑6.59am, £1.48 for 7am‑9.59pm
  • Reporting to: Home Nursing Services Team Leader
  • Pay scheme: Other / Permanent
Key Responsibilities
  • Understand and maintain best practice and regulatory compliance in palliative and end‑of‑life care.
  • Perform the Hospice at Home West Cumbria Initial Home Assessment and alert the senior nurse or out‑of‑hours on‑call of any risk or threat.
  • Continuously assess the patient during the episode of care, responding to changes in condition and seeking advice from out‑of‑hours professionals as required.
  • Assess the need for symptom control and administer medications (oral, injection or syringe driver) as prescribed, reviewing effectiveness and potential side effects.
  • Advise patients and families on symptom control (pain, nausea, vomiting, constipation) to achieve relief from distressing symptoms.
  • Provide families and carers with emotional and psychological support and communicate complex, sensitive information.
  • Verify death and assist with last offices when needed.
  • Attend and participate in supervision and de‑briefings on a required basis, supporting own resilience and wellbeing.
  • Produce accurate, legible documentation of the care given that meets documentation standards.
  • Work as a member of the wider hospice and organisational teams, supporting onboarding of new and existing staff.
  • Act as a Link Nurse or trainer when required and support with HCA clinical supervision.
