Job summary

Jupiter Recruitment is seeking a Regional Live-in Team Leader to carry out care assessments and develop person-centred care plans for clients in their homes. The role covers the East Anglia region with regular travel and is a home-based position.

Responsibilities include responding to enquiries, matching carers, supporting placements and conducting welfare checks. A Level 3 Diploma in Health and Social Care (or willingness to work towards it) and a full UK driving licence are required.

Qualifications

• 2 years’ experience in a care setting with vulnerable adults
• Experience of care needs assessments and writing care plans
• Experience of spot checks, competency assessments and supervisions
• Good knowledge of CQC fundamental standards and safe care at home
• Confident IT skills and use of a digital care management system
• Full UK driving licence and willingness to travel regionally East Anglia

Responsibilities

• Respond to new live-in care enquiries and carry out in-home assessments
• Develop comprehensive, person-centred care plans and risk assessments
• Match the right live-in carer to each client based on needs and preferences
• Support start of new placements, introductions, handovers and first-week checks
• Conduct regular welfare checks and competency assessments with carers
• Build relationships with families and healthcare professionals
• Manage your diary and travel across the region to prioritise urgent assessments
