Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY:

The Heroin Overdose Prevention Education (HOPE) program provides an opportunity for individuals involved with the criminal justice system who are charged with an offense and exhibit substance use to access care and treatment. The long‑term goal of the program is to support participants in managing their lives in the community and prevent future arrests and returns to incarceration.

The Recovery Counselor serves as a key clinical member of the HOPE program team, providing substance use disorder assessments, individual and group counseling, crisis intervention, treatment planning, and recovery support services to justice‑involved participants. The Recovery Counselor works collaboratively with Senior Peer Mentors, court personnel, and community partners to support participant engagement and continuity of care.

  • Facilitate recovery‑oriented skill building and psychoeducational group activities
  • Provide crisis intervention and de‑escalation support to program participants as needed
  • Conduct follow‑up and aftercare calls to support sustained engagement and recovery
  • Coordinate referrals and linkages to treatment, ancillary, and supportive services
