Job summary

Recovery Connections in Darlington is seeking a compassionate Rehab Coach to support people within our Rehabilitation and Aftercare Service. You will work mainly on a one-to-one basis, helping individuals achieve personal recovery goals and build resilience.

The role requires delivering individual interventions, facilitating groups, completing assessments and support planning, and working with partner agencies to promote health, wellbeing and positive lifestyle changes.

Qualifications

• Experience working with vulnerable individuals.
• Experience providing one-to-one support.
• Experience facilitating groups or workshops.
• Experience working in partnership with other agencies.
• Understanding of recovery-focused approaches.

Responsibilities

• Deliver one-to-one recovery interventions.
• Facilitate groups and structured recovery activities.
• Complete assessments and support planning.
• Monitor progress and celebrate achievements.
• Support people through rehabilitation and aftercare pathways.
• Engage with partner agencies to coordinate care.
