Job summary

Change Grow Live in Manchester is seeking a Recovery Coordinator to support individuals through assessment, planning and coordinated care within a multidisciplinary team. The role emphasizes connection, risk management and links to clinical and community services.

You will complete assessments and develop recovery plans, coordinate treatment and collaborate with prescribing colleagues to reduce harm and promote wellbeing in a supportive setting.

Qualifications

• Excellent listening and communication skills.
• Ability to build trusted relationships and have honest conversations about risk and change.
• Experience from drug and alcohol services or related field; strong assessment and case-management skills.
• Organised, comfortable with IT systems and able to work independently within a multidisciplinary team.

Responsibilities

• Complete assessments, recovery plans, risk assessments and regular reviews.
• Coordinate personalised treatment, including clinical and psychosocial interventions.
• Work with prescribing colleagues, health services and community partners.
• Respond to safeguarding concerns and make referrals as needed.
• Promote harm reduction, naloxone, peer support and mutual aid.
• Maintain accurate, confidential records.
