Job summary

Leeds Community Healthcare NHS Trust seeks a Band 3 Healthcare Assistant to support patients in our Short Term Community Beds as part of the Wharfedale Recovery Hub. You will help with personal care, mobility, nutrition and rehabilitation activities to support recovery and independence.

Working with nurses and therapists, you will implement care plans, record observations, maintain documentation, and aid discharge planning in a 24/7 service.

Qualifications

• Provide high-quality care and support to patients in rehabilitation.
• Assist with personal care, mobility, nutrition and daily living tasks.
• Document delegated clinical observations accurately.
• Support timely discharge planning within a 24/7 service.

Responsibilities
