Job summary

Leeds Community Healthcare NHS Trust seeks a Band 3 Healthcare Assistant to support patients in our Short Term Community Beds as part of the Wharfedale Recovery Hub. You will help with personal care, mobility, nutrition and rehabilitation activities to support recovery and independence.

Working with nurses and therapists, you will implement care plans, record observations, maintain documentation, and aid discharge planning in a 24/7 service.

Supporting a Home First approach is at the heart of ourservice. While many patients can return home directly following a hospitaladmission, some require a short-term stay in a rehabilitation setting providing24-hour care and support. Our Recovery Hubs play a vital role in facilitatingtimely hospital discharge and preventing avoidable admissions.

As a Healthcare Assistant, you will support patients withtheir personal care, daily living activities, and rehabilitation programmes,helping them regain independence and achieve their goals. You will work closelywith nurses, therapists, and other healthcare professionals to provide safe,effective, and compassionate care.

Main duties of the job

As a Healthcare Assistant within our Short-Term Care Bedsservice, you will provide high-quality care and support to patients requiringshort-term inpatient rehabilitation. Working alongside nurses, therapists, andother healthcare professionals, you will support patients with personal care,mobility, nutrition, rehabilitation activities, and daily living tasks, helpingthem regain independence and achieve their recovery goals.

You will assist with implementing care plans andrehabilitation programmes, undertake and record delegated clinicalobservations, maintain accurate documentation, and promptly report any changesin a patient's condition. You will also contribute to creating a safe, clean,and welcoming environment, whilst supporting safe and timely dischargeplanning.

A key aspect of the role is motivating and encouragingpatients to maximise their independence and actively participate in theirrecovery. You will work closely with patients, families, and carers to providecompassionate, person-centred care.

This role is part of a 24-hour, 7-day service. The postholder will be required to work a mixture of long day shifts (07:45 to 20:15)and night shifts (19:45 to 08:15), including weekends and bank holidays, on arotational basis. Flexibility to work both day and night shifts is essential.
