Job summary

Leeds Community Healthcare NHS Trust is seeking an experienced Band 6 Senior Physiotherapist to work in our Short-Term Community Beds at Otley. You will deliver assessment, treatment and rehabilitation for adults, supporting early discharge and safe, person-centred care within a 7-day service.

Working in a multidisciplinary team, you will develop rehabilitation plans, provide education to patients and carers, and participate in staff training and service improvement.

Qualifications

• HCPC registration and eligible to practice in the UK.
• Experience in adult rehabilitation and community-based care.
• Ability to travel within the locality for patient home visits or site coverage.

Responsibilities

• Deliver high-quality physiotherapy assessment, treatment and rehabilitation for patients requiring short-term inpatient rehabilitation.
• Work collaboratively with a multidisciplinary team to provide person-centred care.
• Contribute to discharge planning, goal setting and evaluating rehabilitation outcomes.
• Support service development and continuous quality improvement within the Recovery Hub.
• Occasional flexibility with weekend and bank holiday coverage as part of a 7-day service.
