To provide safe, high-quality, person-centred care and support in individuals' homes and the community, promoting dignity, independence, choice and wellbeing in line with care plans, risk assessments, company policies and CQC standards.
Provide person-centred care and support, including personal care, mobility, nutrition, hydration and domestic tasks where required.
Support individuals to develop independence and access social, leisure and community activities.
Administer or assist with medication when trained and assessed as competent.
Provide emotional support, reassurance and companionship.
Monitor and report changes in health, wellbeing, behaviour or risk.
Maintain accurate care records, including care notes and MARs.
Follow safeguarding, health and safety, infection control, confidentiality and other company policies.
Work professionally with colleagues, families and other professionals.
Attend required training, supervision and team meetings.
Work flexibly, including evenings, weekends and bank holidays where required.
Promote dignity, privacy, equality, diversity and inclusion.
Maintain professional boundaries and act with honesty and integrity.
Report safeguarding, health and safety and other concerns promptly.
Comply with relevant legislation, including the Health and Safety at Work Act, Mental Capacity Act, GDPR and applicable CQC requirements.
Undertake other reasonable duties as directed by the Registered Manager.
