Advanced Nurse/Clinical/Paramedic Practitioner

Job descriptionJob responsibilities Clinical Practice Work alongside our GP and ANP partners to manage patients presenting with urgent, on-the-day assessment needs. Deliver coordinated care to "at-risk" patients to avoid unnecessary hospital admissions and ensure they receive treatment in the most appropriate setting. Coordinate clinical care through surgery consultations, clinical triage, and home visits, including proactively supporting patients discharged from acute care. Act as an autonomous clinician, ensuring practice remains safe, effective, and strictly within your clinical boundaries of competence. Examine and assess patient needs from a holistic perspective (physical and psychological), proposing evidence-based care plans. Prescribe safely and competently in accordance with non-medical prescribing standards, verifying the appropriateness of medicines and reviewing clinical indications. Support chronic disease management (such as Diabetes, Hypertension, and Asthma) and participate in clinical audits to monitor and improve the quality of care. Liaising with Patients, Families, and Carers Act as an information resource and focal point of communication for patients, carers, and families regarding their clinical management and care plans. Direct patients and families to external agencies, the voluntary sector, or Social Prescribing services where appropriate. Interfacing with Other Agencies Maintain a strong working interface between Drybrook Surgery, the District Nursing Team, community services, and other stakeholders. Participate in multi-disciplinary team meetings to optimize patient outcomes. Clinical Governance Actively participate in clinical governance processes, including clinical audit, risk management, incident reporting, and CQC compliance. Promptly report clinical and non-clinical safety incidents to the Practice Manager and Partners, contributing to shared learning. Understand, identify, and appropriately escalate any safeguarding concerns for children and vulnerable adults. Adhere to safe systems of work, infection control, and health and safety policies. Managing and Using Information Maintain accurate, legible, and contemporaneous clinical records on the practice's IT systems in accordance with professional codes of conduct and practice policy. Maintain clinical databases of at-risk patients to support service development and contractual compliance. Other Factors A uniform will be required. The role is based at Drybrook Surgery but will involve regular travel across the local area for home visits. Candidates must have access to their own transport and appropriate business car insurance.
