Job summary

Wiltshire Council is recruiting a Reablement Worker in Trowbridge. The role is full-time, on-site, with a salary of £13.92 to £14.36 per hour and a 37-hour week. The post involves in-home support after hospital discharge and close work with occupational therapists to deliver personalised reablement plans.

Applicants should be flexible to a rota covering 7am–10pm, 365 days a year, and must have a valid UK driving licence with access to a vehicle. Disability Confident employer scheme is recognised.

Qualifications

• Training and upskilling through practice and skills development

Responsibilities

• Support individuals in their homes after hospital discharge
• Co-develop personalised reablement plans with occupational therapists
• Travel to customer visits and work a rota 7am–10pm, 365 days a year
